Assessment of radiation exposure and effective dose according to imaging conditions in cerebrovascular interventional procedures

This study systematically analyzed patient radiation exposure during transfemoral cerebral angiography (TFCA) and cerebral aneurysm coil embolization, and quantitatively evaluated organ-absorbed and effective doses using Monte Carlo simulations. To assess dose variation according to projection angles, kerma-area product (KAP) and reference air kerma (RAK) were measured with a clinical fluoroscopic imaging system. Subsequently, organ-specific dose estimation was performed by applying indirect dosimetry with the Korean reference phantoms, enabling the calculation of absorbed doses in 29 organs and effective dose values. The results demonstrated differences in radiation exposure depending on projection angles and procedural conditions, with the highest dose observed during cranial projections. Furthermore, female organ doses were generally higher than those of males, primarily due to differences in body size and radiation absorption. When comparing effective doses based on the recommendations of ICRP Publications 60 and 103, the influence of updated tissue-weighting factors was evident, resulting in higher effective dose estimates. This study provides objective evidence for the quantitative assessment of radiation risk in interventional procedures and offers fundamental data for patient-specific radiation protection strategies and optimization of clinical imaging.
