Methods: This descriptive study utilized secondary data from the Korea National Health and Nutrition Examination Survey (KNHANES) conducted between 2019 and 2021. A total of 386 stroke survivors aged 19 years or older were included in the analysis. Participants were categorized into three age groups: young adults (19-43 years), middle-aged adults (44-64 years), and older adults (>= 65 years). Health behaviors were classified as either health-promoting or health-risk behaviors, and differences were examined by age and sex. Results: Of the stroke survivors, 58.4% were men and 41.6% were women. Men showed higher rates of smoking and alcohol consumption, whereas women demonstrated higher rates of influenza vaccination and Lifestyle-related diseases medication adherence. Significant differences in health behaviors were observed across age groups. Older adults exhibited higher compliance with positive health behaviors, including smoking cessation, abstinence from alcohol, and regular physical activity. In contrast, younger stroke survivors (19-44 years) engaged in more health-risk behaviors, such as lower treatment adherence and increased rates of smoking and drinking. Conclusion: The findings suggest that differences in health behaviors among stroke survivors are more closely associated with age than with sex. Given the prevalence of poor health behaviors and low treatment adherence among younger adults - particularly men - there is a need for age- and gender-specific health education and secondary stroke prevention strategies in community settings.
